Proof of residency is required.  Proof can include:

• Identification with current address (ie, drivers license, state ID card)
• Copy of lease agreement

Also, you must demonstrate financial need and meet program financial guidelines and must not have insurance or inadequate insurance for dental, vision and emergency orthopaedic care or prescriptions.
